• Respond to calls and assist individuals establish therapeutic services in C&F Programs.• Act as liaison to hospitals referring individuals to C&F Programs. • Act as CMHTA liaison for individuals seeking funding for therapeutic services not covered by their insurance and manage a caseload of 2 CMHTA clients. Duties include completing diagnostic assessments with recommendations, establishing therapeutic services for approved clients, facilitating monthly staffings, coordinating care, and maintaining CMHTA documentation. • Coordinate with program managers, therapists, psychiatrists and front desk staff to anticipate, problem-solve and manage challenging/unique client situations.

• Provided weekly case management to a case load of 6-10 clients, 15-20 years old, in a homeless shelter.
