Senior Mental Health Paractitioner
Current• I ensure systematic assessment of the person’s health and social needs is carried out initially, and again when needed (including an assessment of risk and any specialist assessments)• To apply psychological interventions adapted to service user’s needs. (CBT, Motivational interviewing, counselling techniques, Compassion focus therapy). • To Co-ordinate the formulation and updating of the care plan/support plan, ensuring that all those involved understand their responsibilities and agree to.• Building relationships with patients to encourage trust, while listening to and interpreting their needs and concerns.• Mentoring and supervision of Junior staff and students. • Preparing and participating in group and/or one-to-one therapy sessions, both individually and with other health professionals• Responding to distressed service users in a non-threatening manner and attempting to understand the source of distress.• Applying 'de-escalation' techniques to help people manage their emotions and behavior.• Organising social events aimed at developing patients' social skills and helping to reduce feelings of isolation.• Preparing and maintaining service users' records and producing care plans and risk assessments;• Working with service users' families and carers, helping to educate them and the patient about their mental health problems.• Visiting patients in their home to monitor progress and carrying out risk assessments with regard to their safety and welfare.• To ensure that carers and other agencies are involved and consulted where appropriate.Research experience : I have been recently involve in a Q&I project within my team. Project has the aim to gather information from different fonts, process this and to analized data with the aim of implement more standarized process of safety discharge of services users back to primary care from Community services (Secondary Care).
