CREATE INC; West Harlem, NYC. RN Coordinator: Psychiatric/alcohol-chemical rehab: outpatient and residential with homeless/HIV/HepC populations. Health and wellness; Harm reduction education, psycho-social wellness and medication management. Educates as a collaborative process with agency clinical staff and program directors to help participants assume responsibility for their own health,including nutrition/status; practice behaviors that promote wellness, participate in intake medical assessments; periodic assessments and examinations that support early intervention when illness occurs, and assist participants with chronic conditions to lead their lives as fully as possible. This includes a role in developing and coordinating health promoting initiatives, collecting and analyzing health related data and working directly with client and Harlem community liaisons.*+-- Developed materials, tools and strategies to engage program participants in health promotion group sessions in the outpatient and residential (inpatient) facilities. Advise agency leadership regarding health promotion interventions; liaise with external medical resources. Develop policies and procedures to support staff interventions and program and agency wide activities promoting health and wellness.
