Home Care Nurse

Provided primary Care Nursing in an inpatient setting, overseas CNA's, updates care plans, attends IDT (Interdisciplinary Plan Of Care) meetings with pt/family, MD, ARNP,Social Worker, dietitian, Chaplain and contributes in the decision making of the pt plan of care. In addition, be a liaison between the patient and MD/ARNP. Give a daily report, medication administration, reorders meds, make suggestions according to lab values about monitored meds like Coumadin, Digoxin, etc. Monitor pt vital signs, draw lab values and sent to the lab. Follow up with the results by reporting to MD?ARNP. Experience dealing with Hospice patients and their families.Initiating and Updating Care plansPreceptor ship
