Behavioral Health Care Manager
The Behavioral Case Manager is designed to serve customers receiving co-occurring medical, mental,and behavioral services at Bonneville Family Practice. This position is vital to the daily functioning of theprogram and works with a high risk, high need population.My job responsibilities include:• Coordinating care with external agencies, family, payer sources, and other services as needed• Send weekly reports and updates for all patients involved with the courts, AP&P, AMP, DCFS ontreatment progress, UA’s and group attendance• Manage files, review progress notes written from other members of the treatment team• Perform Initial Case Management Substance Abuse Intake• Create individualized care plans to help patient meet treatment end goals/requirements• Weekly/Biweekly/Monthly follow ups with patients• Provide resources, information and referrals to patients as needed• Help coordinate placement for residential treatment• Participate in client-related meetings and trainings• Administer SASSI Questionnaire• Monitor and evaluate patients progress through their case management plan, update and applyrevisions to the care plan as needed.• Prepare case management reports such as: concerns, successes, etc..• Enroll patients into external programs• Actively monitoring Klara for patient communication• Coordinate care that is safe, timely, effective, efficient, equitable, and client-centered• Handle case assignments, review case progress• Help clients achieve wellness and program compliance• Facilitate multiple care aspects (case coordination, information sharing, reporting etc.)• Help patients make informed decisions by acting as their advocate regarding their clinical status andtreatment options• Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
