• Complete intake process for applicants applying for Louisiana long term care programs.• Complete Level of Care Eligibility Tool (LOCET) and other applicable assessments.• Handle incoming and outgoing phone calls to Waiver and Long Term – Personal Care Services (LT-PCS) applicants and recipients.• Provide prompt, professional customer service to ensure that all necessary information is covered and gathered from applicants/recipients in a clear, consistent, and professional manner.• Utilize Xerox and Department of Health & Hospitals’ databases to input and update applicants/recipients’ information and track applicants/recipients’ application process for Community Choices Waiver (CCW), Adult Day Health Care (ADHC) and LT-PCS, as well as PACE program when applicable.• Handle TTD/TTY (telecommunications for the deaf) assistance calls.• Ensure that problems are promptly addressed, documented and rectified on time, with the proper follow-up and professional customer service.• Perform post assessment tasks related to LOCET mailing and follow-up for CCW, ADHC, PACE, and LTPCS applicants/recipients.• Participate in departmental “team effort” functions and projects.• Participate in ongoing departmental and Xerox training activities.• Perform other duties as assigned by management staff.• Make appropriate referrals for non-Medicaid community services.• Document all required information in applicable databases.• Provide consultation and assistance with application processes.• Participate in Continuous Quality Improvement (CQI) process.• Provide assistance with community referrals.

•Process referrals for home health care services after patients are discharged from hospital.•Verifies demographic, insurance, and financial information from referral source, client or caregiver.•Tracks various aspects of care requirements including date of care, supply orders, supervisory visits, certification periods and re-hospitalization of clients.•Monitors and review field staff timeliness with documentation and report findings to management.•Communicate changes in orders to care delivery staff.•Communicates in an effective and professional manner with Physicians, ancillary departments, nursing units, physician’s office staff, insurance companies, as well as clients and their families.•Completes thorough and accurate documentation.•Adheres to all Memorial Hermann Healthcare Systems polices, procedures, and standards, within budgetary specifications.•Adhere to all rules, regulations and guidelines for Medicare and private insurance based referrals.•Provide assistance with community referrals.
