I am an American healthcare entrepreneur and retired lawyer. My partners and I developed and operated a medical clinic network in the United States specializing in rehabilitation medicine in the 1990s. After selling the business in 1995, I traveled to Kenya to discover why 25,000 children in the developing world died each day while the medicine that would save them cost less than a Starbuck's Latte. Many lacked funds to pay, but the greater cause was ineffective, substandard care. I determined that effective-quality care (EQC) could be provided using a basic primary care retail franchise model to establish and enforce EQC standards. So I co-founded The Healthstore Foundation in 1997 to franchise primary care clinics to nurse-practitioners. Since opening the first clinics in 2000, approximately 10,000,000 Kenyans have been served, half in clinics and half in community health education and prevention services. We've also launched a similar network in Rwanda and gave it to a group affiliated with GSK, and another in Ghana which we gave to a group that became affiliated with Sanford Health, America's largest rural healthcare provider for its World Clinic program.With 22 years of experience, I founded a new company, Franchise Labs, to make what we've learned available throughout the developing world. Franchise Labs provides consulting and business development services to clients interested in franchising Whole Person Primary Care services the world's 5,000,000,000 poorest people.

  The Healthstore Foundation operates a franchise network of basic primary care medical clinics in Kenya. They are owned and operated by nurse practitioner franchisees. About 10,000,000 million Kenyans have been served in clinics and communities since opening in 2000. In 2019, the most recent year before Covid, 457,606 people were served: 286,211 in community health education and prevention activities, and 171,395 in clinics where clients paid at average price of $4.50 per visit. Healthstore’s total cost to operate the the franchisor that supports nurse-franchisees' businesses was $165,460; just $.0.36 per client served. Throughout the world, the franchise model has proven to deliver Quality, Access and Affordability--the Three Pillars of Universal Health Coverage (UHC)--in industries where the quality of work performed by workers is mission-critical. It works because franchisee's are required to comply with quality standards as a condition of owning and operating their own businesses; and rigorous compliance programs enforce standards. Franchise chains from hotels, restaurants, petrol/gas stations, lawn services, to home healthcare, and dozens of other industries, all use the same business model because it achieves three things: (1) Each retail location is standardized to deliver a consistent Quality of services; (2) Standardized retail locations can be scaled geometrically by replication to create Access to services; and (3) Economies of scale make services more Affordable. There is no industry in the world where the quality of services is more mission-critical than it is in Global Health, where results are measured in avoidable suffering and death and not just money. Healthstore wants to share its knowledge, experience and tools throughout the developing world so Effective-Quality Care (EQC) can prevail where substandard care causes avoidable suffering and death.
