Practice Nurse

Oversee 12-24 patients and 4-8 patient care technicians. Manage dialysis machines and equipment, initiate and terminate dialysis treatments, cannulate fistula and graft accesses, and provide CVC care and assessment with meticulous adherence to infection control practices. Provide head-to-toe, focused, and neuro assessments, manage IV accesses, hang IV fluid, administer IV, oral, IM, and SC medications, and coordinate care with additional providers. Assess response to treatment and address complications.

Planned and delivered structured home visits for families with children age birth to 5. Provided school-readiness support, education, and developmental screening and surveillance through the Parents as Teachers curriculum. Supported and managed a caseload of 12 families. Referred and assisted families in navigating healthcare, legal, housing, Early Childhood Education, and Early Intervention systems. Conducted medical history & physicals and focused assessments to evaluate patient safety for outpatient surgical setting. Provided patient education on surgical and medication abortion, post-procedure care, and birth control. Provided empathetic and non-judgemental options counseling. Provided direct patient care in PACU and managed workflow between RNs, MAs, and other staff to ensure efficient and safe care in a high-volume, fast-paced setting. Assessed vitals, pain, and bleeding. Addressed clinical presentations of vagal nerve stimulation, uterine clots, and post-procedure hemorrhage with discerning administration of pain management, pharmacology, IV fluids, fundal massage, and activation of emergency response. Performed quality inspection chart audits for discharged patients and coordinated with colleagues to clarify any missing, incomplete, or inconsistent documentation and ensure documentation was accurate and complete.

Performed all front-door screenings, enacted quarantine protocols, educated residents and families, and kept thorough documentation during the COVID-19 pandemic. Communicated with residents, family, nursing staff, EMTS, care assistants, and administration in order to provide complete reports on resident’s medical needs and status while maintaining HIPAA compliance.
