Ensure the quality of medical records by verifying their completeness, accuracy, and proper entry into computer systems. Use computer applications to assemble and analyze patient data for the purpose of improving patient care or controlling costs. Specialize in coding diagnoses and procedures in patient records for reimbursement and research.Conduct concurrent documentation audits of resident health records on a regularly scheduled basis, insuring that information is provided to appropriate individuals and/or department heads for follow-up action/s, based on the results of the audits.Review of all medical records of residents discharged from the healthcare unit following the closed chart order.Storage and retrieval of records both active and closed, based on the mandated retention schedule.Review and processing of health information in response to received request.Inform and track clinical personnel, including physicians who are non compliant with the documentation process.Maintain the daily and monthly census for the healthcare unit, identifying Medicare, Medicaid, HMO's, hospital and private pay days.Review and update Medical Records policy and procedure manual at least annually.Maintain secondary indexes as mandated; admissions, discharges, re-admissions, hospital stays.Perform other appropriate activities as designated.
