Seán is a values driven entrepreneurial physicists passionate about artificial intelligence, honed from almost two decades of experience creating and capturing value from real world data. He is a recognized international expert in healthcare and is now also exploring the defense and climate domain spaces for opportunities. Issues include the rapid development and use of novel technologies and treatments, the difficulties of randomised clinical trials (RCT) in this area and the relatively small numbers of RCT patients as a proportion of the whole patient population (2-3%).However, there is a huge amount of routine clinical and imaging data collected on cancer patients that could potentially provide further evidence, complementary to RCT data, to help support optimised clinical decisions. Besides patient, treatment and outcome data, a large image-based dataset is available from routine radiation oncology for lung cancer, with (4D-)PET-CT and cone beam CT increasingly being used in addition to the standard practice of acquiring diagnostic, treatment simulation and regular follow-up CT scans. By bringing imaging and other clinical data together from a range of cancer centres, a much stronger (Big Data) dataset can provide more soundly based decisions.This proof-of-concept project hypothesises that 1) for lung cancer radiotherapy (RT) it is feasible to harness data from NSW cancer centres in a data-protected manner, that 2) bioinformatics can be used to interpret this data and provide meaningful decision support and that 3) the addition of radiomics (imaging-based) features can strengthen the clinical decision process. During the project, data on lung cancer patients, treatments, imaging and outcomes from uncoded sources from multiple NSW centres will be extracted, standardized and linked; at the same time keeping each centre’s data secure within its own protected systems. Bioinformatics tools will be deployed to learn and validate in a distributed manner an outcome prediction model for survival in NSW lung cancer patients, predicting whether a patient is expected to benefit from radical radiotherapy. Ongoing work involves the development of models using advanced data mining and image analysis techniques applied to lung cancers to customize the treatment for individual patients. This research program on multi-factorial prediction and Decision Support Systems is embedded in the research institute GROW and the Faculty of Health, Medicine and Life Science at the University of Maastricht. It is envisioned that that prospective clinical trials will validate the predictors and test their applicability. Our work within the ENVISION project is a joint venture between the Maastro Clinic and the Gray Institute for or Radiation Oncology and Biology, with close collaboration of the Medical University of Vienna: Division of Medical Radiation Physics. The potential clinical effect on the dose distribution due to errors in beam delivery, patient changes, etc, is a subject of great interest. Emphasis is especially focused on the prediction of treatment outcome through radiobiological models based on these dose distributions and the clinical effect of these errors.
