CurrentCase management for Colorado's underserved population- homeless, justice involved, IV drug users and adults with chronic conditions. Outreach to local shelters and re-entry service providers to ensure these populations receive medical care and other supporting services. • Assisting in identifying risk factors leading to increased utilization while working to reduce preventable hospitalizations, readmissions and inappropriate utilization• Performs advanced clinical assessments and formulation of care plans for complicated, co-morbid members, while assuring appropriate and complete data entry of interactions with members, providers and other stakeholders.• Established relationships with community providers of a hard to reach population [homeless].• Created HIV/HepC “scripts” for special populations that will service as a guideline for additional special populations scripts. Working with members to promote self-efficacy in overall well-being, confidence and abilities to manage personal health care needs. • Managing a substantial case load of chronically ill members while collaborating with a multi-disciplinary team, employing a variety of strategies, approaches and techniques to enable each member to manage their physical, environmental and psycho-social health issues.• Strong understanding of HIV, HepC and other chronic conditionsWell established professional relationships with primary medical home providers
