I work 5 clinical sessions a week as an autonomous ACP where I manage my own patient caseload, with weekly on-call and triage duties, provide home visits, and also responsible for the mentorship of other clinical colleagues including junior doctors, and medical students. Also as a partner at the practice I have clinical and administrative responsibilities that cover the running of a safe, efficient and profitable GP practice.

I have worked as an ACP and senior clinical pharmacist at Colne Valley Group Practice for several years. My daily responsibilities include seeing patients with acute presentations, chronic disease management, and prescribing management at the practices.Here I fully developed and implemented a clinical pharmacist service, and embedded a structured appointment system which catered for both telephone and face to face appointments. This allowed for the development of a more efficient and effective clinical pharmacist service by adjoining the role of prescribing management with patient facing clinical work.I have also trained and supervise a team of pharmacy technicians at the practice to support the prescribing management work and process discharge and clinical letters. This has been a very successful and productive change which has resulted in freeing up valuable time for increased patient facing work.

For the past two years have been the lead pharmacist for the Valleys PCN where I have had the responsibility of developing a new PCN pharmacy team. This included implementing a new pharmacist service for the PCN, mentorship of the new PCN pharmacist and pharmacy technicians.

I work at Greenhead Family Practice as a clinical pharmacist and independent prescriber. Here I manage my own clinics with the responsibility of an independent patient caseload in the areas of minor illness and chronic disease management. Part of my clinics are during the practices extended access service and therefore are provided outside normal practice working hours.My other responsibilities include conducting clinical medication reviews with patients; poly-pharmacy reviews; managing the repeat prescribing reauthorisation process; reconciliation of medicines in outpatient and discharge letters; assessing acute medication requests; and answering medicines information enquiries from the GPs and other healthcare professionals and patients.

At Sunnybank Medical Practice I worked as a GP practice pharmacist and independent prescriber. Working over four days a week my primary role was managing patients with long term chronic conditions in the areas of hypertension, diabetes, asthma, COPD, depression, pain, epilespy and other chronic conditions. This was done in fixed daily sessions through face to face appointments and telephone reviews. This role also involved taking physiological observations such as BP, pulse and requesting pathology tests.My specific responsibilities included conducting clinical medication reviews with patients; poly-pharmacy reviews; managing the repeat prescribing reauthorisation process; reconciliation of medicines in outpatient and discharge letters; assessing acute medicine requests; and answering medicines information enquiries from the GPs and other healthcare professionals and patients. I was also involved in helping to achieve and deliver the QIPP and QOF targets for the practice and conducted numerous clinical audits in areas such as high risk drug prescribing, epilepsy and valproate.This role required me to have excellent time management skills, as all my appointments were pre-booked and conducted within a slotted appointment time, 15 minutes for face to face appointments and 7.5 minutes for telephone appointments.

I worked for over six years as a branch manager in a community pharmacy situated within a GP health centre. This was a new branch without an existing patient base which I developed into a successful and profitable business through hard work and commitment. I developed a high repeat dispensing business with a strong focus on providing a high standard of advanced services through the provision of Medicine Use Reviews and the New Medication Service. The branch has a high percentage of Electronic Prescriptions due to the focus I placed on working to retain our patient base, which was reflected in a strong EPS nomination of 118%, which was a company wide high.Through this role I developed a mutually beneficial relationship with the GP practice for whom I went onto provide services as a practice pharmacist. This allowed me to provide my patients with an unparalleled level of clinical service and also helped support the practice in its objectives and goals. Through building this strong relationship and by becoming an integral member of the surgery team I strengthened the pharmacies position.My personal ethos was to provide a patient centred service with a strong focus on high clinical and professional standards. This was reflected in the strong relationship that I developed with the GP practice and through my own continued professional development having undertaken further postgraduate education.

In this role I was responsible for various surgical and medical wards providing clinical pharmacist support. This included work on the paediatrics, gynaecology and obstetrics wards. I successfully integrated into both my wards and the department to become a valued member of each of the respective teams. I also worked in the busy outpatients dispensary where I worked alongside a large team of pharmacy technicians and ACTs to process a high volume of outpatient and inpatient prescriptions. As the hospital is the Regional Infectious Disease unit I had the opportunity to widen my clinical knowledge and experience of the medications used in the treatment of HIV, Hepatitis B and C, Malaria and other tropical diseases. I also covered mental health wards, which gave me an insight into the pharmacological management of mental health patients. This role also included the responsibility of clinically checking all clozapine prescriptions for both inpatients and outpatients.

I undertook various roles within the department, ranging from answering medicines information queries due to the complex medication regimes to dispensing against strict prison guidelines and policies. I quickly became an integral member of the multi-disciplinary team by providing high-level support to the wings and medicines management training to healthcare staff. During my tenure I became proficient in using SystmOne to electronically dispense medication and was involved in delivering an Integrated Drug Treatment System within the prison.
