Responsible for coordinating and overseeing development of comprehensive patient safety improvement initiatives to reduce medical errors and safety incidents. Work collaboratively with local leadership, nurses, physicians, and executives to develop strategies to increase patient safety. Safety Areas of expertise include: quality and safety reporting, root cause analysis, process improvement, population health, hospital-acquired conditions, and clinical effectiveness.- Developed and designed a Primary Care Provider Resource for families and a supporting workflow to increase percentage of asthma patients discharging with a PCP and an appointment made from 31.6% to 50.59%- Conducted Failure Modes and Effects Analysis with multi-disciplinary stakeholders to understand severity, occurrences and detections of failure associated with implementation of an automated code blue response. Facilitated hospital-wide new activation process utilizing middleware technology and mitigation of highest risk points.- Conducted several root cause analyses leading to safety improvements throughout the organization including implementation of checklist item for pre-transport to CT for trauma patients (100% compliance), increasing detection systems for retained foreign objects, and advocacy for capital purchase of airway safety equipment.
