Assist seniors with transportation, medical appointments, healthcare advocacy, care coordination, medication management, errands, shopping, light housekeeping, organizing, cooking, hospice, and pet care. Incorporates a holistic approach to improve quality of life focusing on individualized interests and abilities.

Responsible for conducting comprehensive needs assessments for elderly clients referred for case management including development and implement service plans, coordinate and monitor the delivery of services requested in the service plans, develop goals and evaluate outcomes, facilitate quarterly meetings with clients and their families, provide resource information and act as an advocate for clients and their family members.

Responsible for the Supported Community Living Program which provided services to mentally ill adults promoting independent living. This included recruiting, interviewing, hiring, coordination of training, and termination of all employees. Facilitated weekly meetings with administrators and clinical staff to promote best possible outcome for customers, facilitated intakes and yearly staffing meetings with interdisciplinary team members and customers to determine service plans and goals. Also responsible for all client documentation and chart content to be in compliance with Chapter 24 guidelines.
