Guiding the clinical quality improvement work across the ambulatory practices, both primary care and specialty, for this regional health system, covering a 17 county area in Western North Carolina. Working with a multidisciplinary team to bridge the gap between inpatient and outpatient health care and improve transitions of care. Using Lean Six Sigma tools, creating Care Process Models to improve clinical quality, reduce unnecessary variations in care, and achieve the quadruple aim. Partner with regional practices in the ACO to share clinical quality tools.
