Complex L D cases including dual diagnosis, forensic, challenging behaviour and transition. Working with Service Users, families, carers and multi disciplinary professionals. Completing assessments and support plans, Carers assessments and support plans, Mental Capacity Assessments, Ministry of Justice reports, Court of Protection applications, Forced Marriage Protection Order applications and best interests decisions. Attending Care and Treatment reviews, school reviews and health care and education plan reviews.

Multi disciplinary team comprising Social Worker, Targeted Youth Workers, children's centre worker and therapeutic play worker. The aim of the teams was to work with C I N families to avoid their situation escalating to C P. The team worked closely with schools, nurseries, C A M H s and other educational establishments. The team was able to work intensively with families, health and education to establish robust and sustainable intervention strategies. We also provided parenting courses on a regular basis working not only with those carers with whom the team were already involved but the wider community also.

Working with children 14+: on the autistic spectrum, with mild/moderate L D, physical disabilities or life limiting conditions planning and managing their transition into adult services.

Joint funded health and social care project working with patients, G P s. hospitals and families to provide pre and 6 week post operative health, social care and equipment support to avoid cancellations due to treatable health conditions and delayed discharges. Comprising a day centre primarily, but not exclusively, for Afro Caribbean elders. Dom agency contracted to L A to provide support to local ethnic minority communities. Training agency providing N V Q courses (levels 2 to 4) in I T, care and management. I was Internal Verifier and centre coordinator for these courses as well as providing lecturing in these subjects and certified courses in manual handling, risk assessment and risk management . The project was also involved in providing 'life long learning'. My remit also included researching and submitting funding bids to the E S F and E R D F, setting up local joint funding initiatives within the local African Caribbean communities, preparing the annual report and presenting it at the A G M. I was responsible for 100+ staff, strategic planning and development, managing finances and developing community involvement in the project.
