I am telephonic Medical Case Manager for injured employees of School Board, View claims, work with the injured employee, the Drs and the school board to help the employee get back to gainful employment.

Started as clinical manger in Hermitage, Tn, changed to become case manager in field to visit pt's in their homes in January, 2011; office in Hermitage closed; transferred to Palatka Amedisys office as case manger, from July to September. Took some time off.

With Amedisys Home health company I was a field case manager in which I manage the care of my patients, educating, assessing and monitoring them, act as liaison between pt and their doctors, supervising LPN's and Home Health Aide, prior to this position I was Clinical Manager in which I act as clinical manager 50% of the time and field staffs nurse the other 50%. I take call one week night each week In the field I have managed IV therapy, assessed and taught on conditions such as CAD, CHF, COPD, DIABETES, AND WOUND CARE, included supervisory visits, and assessments with starts of care, resuming care post hospitalization, recertification's and discharge.

In this position to this point, I have had to be very flexible from reviewing nurse's assessments, and orders, to taking physician orders, to holding team care conferences with the director of operations. My duties include telephonic Care manager( Pt education, assessment, and referral) for Core Conditions of Cardiac (CAD and CHF), Respiratory (Asthma and COPD) and Diabetes. When a new Team for Condition Care Support was formed I was chosen to be on that team. My duties include:Using Critical thinking skills to educate the members in the standards of care and the guidelines for those with High severities with the core condition and the condition support diagnoses, communication with physicians, and referrals to appropriate sources, while moving the patient toward appropriate outcomes with their conditions. The condition Support platform includes: Hypertension, Metabolic Syndrome, Hyperlipidemia, Arthritis, low Back Pain, and any of the cancers. Many of them did not know that their insurance would cover this, I was able to enlighten them on what the test would reveal, and how it could actually prevent further problems for them as well as save them and the insurance company money in the long run.
