Hackensack Meridian Health Carrier Clinic’s East Mountain Youth Lodge provides a continuum of out of home residential treatment services for adolescents between the ages of 13-18. We provide 24 hour clinical, therapeutic treatment for emotional, behavioral, psychiatric and substance abuse difficulties based on the resident strengths and needs. Primary Responsibilities: Residential Counselors uses a nurtured heart approach to provide supervision, behavior management, and counseling of the basic needs for the youth, Residential Counselors will build relationships and assist with developing our adolescence through support of behavior modules. Responsibilities include leading resident group meetings, coordinating recreation activities, and participating in other group functions. BUTTERFLY LODGE: A population of adolescent females with more of a mental health focus of treatment and more intense co-occurring substance abuse disorders. These youth more frequently struggle with depression, anxiety, psychosis and self-harmful behaviors including self-mutilation and/or suicidal ideation, gestures or attempts. EAGLE LODGE: A population of adolescent females with a behavioral focus to treatment. These young ladies struggle most prominently with oppositional, conduct disordered behavior including anger/ aggression and co-occurring substance abuse disorders.SUNFLOWER & LOTUS LODGES each serve 5 adolescent females who receive intense mental health treatment congruent with the regulations and expectations of a Psychiatric Community Home. Both programs embrace Wellness and Recovery. SWAN LODGE: represents the highest level of intensity of out of home treatment programming and is a co ed population of 12 young men/women who have transitioned from various inpatient settings who need intense treatment services before returning home or to less restrictive settings. Swan Lodge residents present with the highest level of acuity/need within the state system of care.

Venture House is a community based mental health agency, utilizing the Clubhouse model of Psychosocial Rehabilitation. Our mission is to support individuals living with mental illness to achieve recovery through improved access to education, employment, recreation, housing, and community engagement in an environment of mutual acceptance and empowerment. Despite the word “house” in our name, we are not a congregate care facility, and no one lives onsite. Primary Responsibilities: Engaging members in the “Virtual Clubhouse” through video conferencing,phone calls, creative projects, letters, virtual group facilitation, participation in a Facebook group, housing program assistance, and other tasks as assigned. There may be situations where a hybrid schedule is required. Under normal circumstances, Staff work side-by-side with Clubhouse Members (program participants) to run day-to-day operations, including intake and admissions, meal preparation and service, facility maintenance, program development and implementation, job development and coaching, as well as social and recreational programming. The primary responsibility of a psychosocial rehabilitation worker (normally) is to engage Clubhouse members, encouraging their participation in the “Work Ordered Day” (WOD). Psychosocial rehabilitation workers assist our members in reaching their recovery goals, including housing; employment; education; financial literacy; socialization; sobriety; and wellness. Psychosocial rehabilitation workers are also responsible for: job development, job coaching, benefits advising, case management, crisis intervention, day-to-day operations, apartment visits, and service documentation.

Primary Responsibilities: Provide information and referrals for various community resources and programs. • Conduct a full benefits eligibility screening: explain eligibility rules for public benefits, assist with enrollment and/or recertification, and assist with the appeal process.Provide person-centered options counseling to consumers by supplying appropriate and comprehensive information about their long-term care options for LTSS services. Ensure confidentiality and HIPAA compliance is maintained for all consumers who use the Program, and comply with CIDNY policies on privacy and data protection.Participate in NY Connects training and continually educate oneself on resources and programs benefiting those with disabilities. Attends the agency's in-service training and unit meetings as well as any other agency-related activities. Perform other program-related duties as assigned.
