Maintain visibility to residents, staff, and visitors by wearing a safety monitor vest, ID badge, and professional attire.Screen all clients and visitors entering the facility.Perform bag searches upon intake and as required.Ensure clients abide by the facility, DSS, and OTDA policies and procedures.De-escalate potential crisis situations and document all interventions.Conduct rounds throughout the facility including hallways, stairwells, floors, bathrooms, and other common areas.Complete logs, incident reports, and all other required documentation.Perform administrative and receptionist duties at the operations/front desk.Monitor video surveillance cameras, fire prevention, detection alarm, and notification system.Support the running of fire drills; evacuate the building when necessary, utilize fire safety procedures.Assist with room preparation and turnover for new clients.Maintain inventory of cleaning and office supplies and ensure proper distribution.Inform management of work site’s physical condition (i.e. dangerous conditions, possible repair needs, supply needs, etc.).Support afterschool/evening activities as needed.

• Maintain a caseload of approximately 25 families. • Provide case management services; including, but not limited to advocacy and counseling, to both the primary client and other family members residing at the New Broadway shelter. • Assist with Independent Living Plan development in order to prepare for permanent housing. • Assess clients’ needs with particular attention to family dynamics, mental health, and overall aspect of their wellbeing. • Provide care coordination with medical and other social service providers. • Assist with group facilitation in the areas of treatment adherence; supportive; mental health; activities of daily living; and substance abuse. • Provide entitlement and medical escorts as needed. • Conduct regular unit inspections. • Counsel families on housekeeping skills. • Submission of timely monthly documentation. Other Responsibilities • Attend divisional housing meetings, weekly staff meetings, quarterly agency meetings, and other internal staff meeting as required. • Appropriate and timely referrals on behalf of clients, provides client advocacy in negotiating systems and delivery of services. • Attend and/or coordinate case conferences with other providers. • Other duties as assigned.
