I coordinated and discharged patients from the Psychiatric Urgent Care unit at the brand new Sheppard Pratt Baltimore/Washington Campus in Elkridge, Maryland. Psychiatric Urgent Care is specially designed for people who need an immediate psychiatric assessment and a referral to an appropriate level of psychiatric care. All individuals are evaluated by a licensed clinician, voluntarily, who is medically stable, free from drugs or alcohol, and able to safely wait for evaluation in an outpatient clinic setting.- Collected psychiatric, demographic, and insurance data.- Coordinated with the psychiatric nurse practitioner to transition the patient to an in-patient unit, day hospital program (PHP), outpatient services, or other agencies, as necessary.- Coordinated with intake, admissions, and other Sheppard Pratt staff to facilitate the transition of the patient from urgent care to the appropriate behavioral health services. - Actively communicated Sheppard Pratt programs and services to internal and external groups. - Completed required written and online documentation requirements for admissions and referrals.

The tasks of this position at this location included what would usually be broken up among multiple roles (e.g., service coordinator, discharge coordinator, medication monitor, etc.). - Trained in performing a review of referrals to the crisis house (RCS) and making the decision to accept or decline the referral, including Optum authorization.- Created the referred client's profile in the electronic health records database.- Coordinated with the local pharmacy for filling medications and the referral community agency to transport the client. Transports clients and completes new client intake paperwork. - Performed new client psychosocial assessment, suicide screening (C-SSRS), and individual treatment plan.- Assisted clients with learning and practicing their independent living skills, social and communication skills, and maintaining the rules and guidelines of the residential crisis house.- Supported clients with local resources, online research, filling out housing applications, and coordination with their local and telehealth service providers. - Wrote daily contact and progress notes for each client pertaining to their behaviors, interactions, goals, and mental status.- Offered various interventions to the client to assist them with their self-regulation and personal goals within my scope of practice.- Facilitated client groups and meditation sessions and offered support and resources as appropriate.- Provided crisis interventions, suicide screening (C-SSRS), safety planning, and assisted with the emergency petition process, as necessary.- Supported clients in managing medications and complying with medication regimens.

Social work intern on the assertive community treatment (ACT) team for Sheppard Pratt at Howard County in Columbia, Maryland (formerly known as Way Station Inc.) providing the following services within my role:- Provided rehabilitation support, skills training, case management, and crisis intervention througha trauma-informed lens as an integral part of the assertive community treatment (ACT) team.- Maintained contact notes after client interactions, performed individual treatment plans (ITPs), andparticipated in morning team meetings with ACT staff.- Shadowed the psychiatric rehabilitation program, the substance use counselor, and individual therapy sessions and assessments through the outpatient behavioral health clinic.- Participated in training for HIPAA, person-centered planning, motivational interviewing,solution-focused brief therapy, suicide screening, and crisis management, among others.The ACT team is a mobile community outreach program that helps people who experience severe and persistent mental illnesses (SPMI) through behavioral health services to improve their overall health and wellbeing while connecting them with resources at the local, state, and federal levels. As an intern, I shadowed various practitioners from vocational coordinators to registered nurses to licensed social workers while also learning about other programs that Sheppard Pratt offers and continued training through the Relias system.

As a clinical intern, I shadowed and worked alongside licensed social workers to perform various daily tasks in an addiction recovery treatment program offering group therapy and case management for intensive outpatient (IOP) and outpatient (OP) services along with individual therapy. Some duties included:- Provided case management services that supported a positive treatment outcome.- Served as therapist intern in IOP and continued care groups as well as provided individual therapy as needed for SUD and/or mental health disorders.- Organized and makes educational presentations related to behavioral health and recovery.- Conducted biopsychosocial assessment, SNAP inventory, and brief addiction monitoring (BAM) summary.- Developed patient-centered treatment plans based on results from various screening tools and bio psychosocial assessments.- Collaborated with medical staff and treatment team as a whole. - Consulted with family members and offers family sessions with the patient.- Coordinated with referral sources and other patient supports to assist with patient care, as appropriate.- Ensured adequate case management services are identified and addressed.- Responsible for the maintenance of electronic health records which include documentation of treatment activity and progress.- Ensured that patient records are accurate, comprehensive, and met regulatory standards.

As a psychiatric rehabilitation program intake coordinator, I provided assessments and next steps for children, adolescents, and adults from the local community for counseling and behavioral health services.- Assessed clients referred to the psychiatric rehabilitation program (PRP) for behavioral healthchallenges, including suicide screenings (C-SSRS) and rehabilitation goals.- Demonstrated adherents to confidentiality and mandated reporting laws and regulations.- Reviewed diagnostic assessments from referring therapists.- Coordinated with referred clients, their families, licensed therapists, PRP counselors, and other agency staff, as appropriate.
