Personal in-home Caregiver

*Supervise and monitor caseload of (100+) one hundred Parolee clients* Provide case management for program participants*Develop and implement comprehensive treatment plans* Conduct comprehensive Mental Health and Alcohol and Drug assessments/evaluations and prepare written summary of findings*Provide treatment recommendations and ongoing monitoring and evaluation of participants* Assist participants in navigating through the program*Assess and identify clinical issues and create clear goals and objectives for treatment plans*Track and document clients progress in weekly and monthly case reports*Create, update, maintain and monitor case records*Conduct individual counseling sessions weekly *Facilitate referrals to treatment providers for case management plans to outside networks*Provide referrals for clients to inpatient programs (STIRRT and IRT)* Establish, maintain, and manage relationships with key partners*Coordinate volunteer participation* Recommend clients for specific programs based on treatment AOD assessment findings* Develop continuing care treatment plans*Document scheduled meetings and provides crisis interventions*Assist Intake Coordinator staff with urinalysis tests*Provide urinalysis tests one weekend bi-monthly *Conduct treatment plans with DOC parole officers and treatment providers for referrals* Back-up intake coordinator * Data entry into Peer Assistance Services program data base* Additional duties as assigned by the Peer Assistance Services-TASC Program Director*Train staff and serve as a member of QM Team
