The project focused on Patient Safety related to effective incident reporting. The project has provided a new and completely revised incident reporting tool which was the result of an extensive literature review and customization of published tools in the industry. With a more robust tool Adagio Health, specifically the Quality Department, can review, categorize, and plan effective follow up action based on clearly identified problems that impact patient care outcomes. Administrative, clinical and operational problems can now be resolved and effective plans of action put in place to provide strong safety measures. An Incident Report Review Committee, a committee of Adagio Health staff and clinicians, was established for evaluation and input regarding process breakdown, CQI, and best practices.
