Registered Nurse: Clinic For Malignant Hematology
Current* Triage patients over the phone and coordinate care. Inform providers of patients' urgent health needs. * Coordinate client needs for home health needs, social work, well visits, occupational therapy, IV therapy at home among other needs. * Maintain patient confidentiality and safety * Obtain thorough medical history * Verify and reconcile medication records* Review blood work and coordinate chemotherapy regimen* Assist patients in compiling list of concerns to discuss at next physician appointments to empower patient/family in their care* Coordinate imaging necessary for diagnosis and treatment of cancer or identify progression of disease* Provide accurate nursing assessment, diagnosis, planning, intervention and evaluation.* Obtain records of patient testing from outside facilities (slides, imaging, blood work, MD notes)* Collaboration with other disciplines when referrals are made: gastroenterology, pulmonary, gynecology, nephrology, psychiatry, dietary counsels * Patient education on disease, medications, chemotherapy, plan of care, treatment* Ensure all interventions are documented appropriately and in a timely manner
