Had a clientele of approximate 10 to 15 Patients at any one time. Case Management and direction of Ancillary staff as well for all patients and following CMS guidelines for documentation. I acted as a liaisons between patients, their families and health care personnel to ensure efficient and prompt health care service. Responsible for devising care plans, which include patient referral or admission, medication doses, administering therapies and treatments, evaluation of results and a summation of the plan's effectiveness.
