-Supported and provided care for 12-year-old and adults with developmental disabilities in all aspects of the community living/community inclusion-Instructed, observed, monitored, and assisted individuals, at the level required to maintain a healthy and safe environment, in basic independent living skills-Implemented programs to meet goals and objectives as stated in the Person Center Action Plan (PCAP)-Aided with significantly challenging behaviors and followed Behavior Management Plan as specified in PCAP, recorded data and turned in as required
