Clinical Social Work Associate
Current-Facilitate referrals from interdisciplinary team members in order to provide community resources and support for families of children facing significant developmental challenges-Conduct biopsychosocial needs assessments to gather information necessary to develop an appropriate plan of care -Provide supportive counseling for families in order to help manage stress, anxiety, fear, and grief -Collaborate with community partners and medical team members (within PCDI, specialty clinics, PCP, etc) to improve health outcomes and build a network of support for families-Contribute knowledge of trauma informed care to clinic providers and utilize this approach in working with families -Obtain clinically significant history in monthly diagnostic clinic for school aged children alongside developmental pediatrician, psychologist, speech and occupational therapy-Document all interactions in the electronic medical record and maintain patient confidentiality according to HIPPA guidelines-Receive supervision and obtain clinical hours in pursuit of Clinical Social Worker Licensure (LCSW)-Created and presented presentation for health care providers on ways to navigate difficult conversations with patients and families
