Responsible for the complete, accurate and timely completion of patient medical records, including prepping, scanning, and auditing and analyzing medical records, both quantitatively and qualitatively, documenting and tracking deficiencies for both physicians and allied health staff, and reporting results deficiencies directly to both the CEO and CCO as necessary. Other responsibilities include pulling, maintaining, filing, and auditing medical records, release of information, and monitoring the documentation in the medical record to make certain it meets hospital policy, and HIPAA regulatory requirements.

Performed initial quality review of medical records for compliance with HEDIS mandates. Sorted medical records according to medical review measure and entered tracking information into Access database. Called physician offices for missing information, faxed out and mailed medical record requests for information. Created reports to assist with review tracking and maintained organized filing system. Trained co-worker in MS Access tracking database.

Performed quality review of medical charts during prepping, scanning, and indexing of records for entry into Horizon Patient Folder of Meditech Module database.
