The purpose of the Homeless ID Project is to assist the identified target population with accessing behavioral health services, medical, entitlements, i.e. SSI or SSDI, or other community supports. Birth certificates and identification are often critical documentation that is needed to gain access to other services. The goal of this project is not only to provide birth certificates and identification cards but to assist individuals with accessing other support to move them from homelessness to stability. Served as the Homeless Coordinator for the MD Behavioral Health Administration's Continuum of Care (formerly Shelter +Care prior to 2012) Housing Program. The Continuum of Care (CoC) Program provides permanent housing in connection with supportive services to homeless individuals with disabilities and their families. The primary target population consists of homeless people that have a serious mental illness, or comorbid chronic problems with alcohol, drugs, both; and /or Acquired Immunodeficiency Syndrome (AIDS) or related diseases. Behavioral Health Administration's CoC Housing Program was designed as the housing component of the Maryland Community Criminal Justice Treatment Program.
