Visit with the client either within their home or in the community as per requirement for services. Conduct service coordination in a manner that supports the client's independence and freedom of choice. Collaborate with the client, treatment team, and other agencies or programs to assist the client with achieving their desired goals. Engage clients to develop and maintain therapeutic relationships. Assist clients with establishing daily structure so they may be confident in their ability to complete daily living activities (DLA). Assist the client in developing the skills necessary for successful functioning in the community. Assist the client with securing all entitlements and benefits they are eligible for and monitor eligibility status for outside agency-mandated periodic reviews or renewals. Promote community resources for the client's community integration. Responsible for developing interventions for services and supports deemed medically necessary as reflected in the client's person-centered individual rehabilitation plan (IRP). Complete contact notes, service tickets, and monthly summaries.

Oversaw all hiring stages from sourcing to onboarding. Sourced and recruited candidates by using databases, social media, etc. Screened candidate's resumes and job applications. Conducted interviews using various reliable recruiting and selection tools/methods to filter candidates within schedule. Assessed applicants’ relevant knowledge, skills, soft skills, experience, and aptitudes. Acted as a point of contact and build influential candidate relationships during the selection process. Assisted in maintaining an established applicant tracking system.

Providing oversight & coordination of the new statewide Homeless ID Project established through the Alcohol Tax Appropriation during 2012 legislative session. Work closely with the lead Regional Core Service Agencies to administer the project.The Behavioral Health Administration (BHA) will provide funding to the Core Service Agencies (CSA) to contract with community providers to pay the cost for Maryland birth certificates and state identification for individuals who are homeless or at imminent risk of becoming homeless, and have a mental illness or co-occurring substance use disorder. The purpose of the Homeless ID Project is to assist the identified target population with accessing behavioral health services, medical, entitlements, i.e. SSI or SSDI, or other community supports. Birth certificates and identification are often critical documentation that is needed to gain access to other services. The goal of this project is not only to provide birth certificates and identification cards but to assist individuals with accessing other support to move them from homelessness to stability. Served as the Homeless Coordinator for the MD Behavioral Health Administration's Continuum of Care (formerly Shelter +Care prior to 2012) Housing Program. The Continuum of Care (CoC) Program provides permanent housing in connection with supportive services to homeless individuals with disabilities and their families. The primary target population consists of homeless people that have a serious mental illness, or comorbid chronic problems with alcohol, drugs, both; and /or Acquired Immunodeficiency Syndrome (AIDS) or related diseases. Behavioral Health Administration's CoC Housing Program was designed as the housing component of the Maryland Community Criminal Justice Treatment Program. The goal of the housing program is to break the cycle of recidivism for individuals who end up in the criminal justice system as a result of being homeless.

Conduct residential site inspections for behavioral health Residential Rehabilitation Programs (Alliance, Inc. & Key Point Health Services), assist with the management of entitlements, provide housing & service referral information, assist with referral review & monitoring of residential waiting list , assist individual & providers with obtaining affordable housing & to participate in the development of supportive & affordable housing choices.

Oversee the clinical supportive services provided to S+C participants. Assist Director of S+C Housing Program by authorizing new and existing participants. Provide resolution to S+C Housing related issues, as well as provide technical assistance regarding supportive services match and the development of participant service plans. Monitor local compliance with S+C regulations by conducting Housing Quality site inspections and site visits to programs to monitor contract deliverables. Assist with maintaining participant files in compliance with HUD guidelines. Assist with the development of policies for the S+C Program and other specialized programs targeted to those who are homeless, coordinate housing quality standard trainings and other homeless training or conferences to support services for persons who are homeless. Provide consultation to Core Service Agencies (CSAs), local providers, and case managers. Assist with preparing federal grant applications for HUD funding, annual reports, and budgets. Participate on statewide and local continuum of care planning boards to address homelessness and apply.

DHMH Medical Assistance Personal Care Program, Edgewood, MDProvided assistance to individuals with physical, developmental, and mental health disabilities and/ or are chronically ill and need help with daily living activities allowing them to remain in the community and out of a long-term care facility. Provided personal care, including bathing, skin and nail care, ambulation, feeding and light meal preparation, transfers, bed making, light housekeeping, and toileting.

Facilitated the implementation of ADL skills to a maximum of 7 individuals diagnosed with a severe and persistent mental illness under the DHMH Project Home Adult Foster Care Program. Monitored medications and maintained medication orders. Coordinated medical and psychiatric appointments for the residents. Meal preparation. Developed and facilitated recreation activities for residents. Adhered to state and local regulations.

Linking eligible clients to community resources to meet their individualized needs including, but not limited to, securing entitlements, mental health, substance abuse treatment, medical, financial, educational, vocational, residential and social support services. Worked closely with a treatment team and families of adults who have serious and persistent mental illness. Verified Medicaid insurance eligibility through EVS and secured APS authorization for eligible clients to receive mental health services. Developed short-term/ long-term goal plans to assist the client to maintain independence and psychiatric stability in the community. Assisted with workforce development job readiness skills when applicable. Assessed new clients and annually re-assessed clients still participating in the program. Responsible for documentation of all services provided. Managed specialized caseload of Shelter Plus Care Housing program voucher recipients.
