I served as the hub of communication for the AL department; acted as the front-line customer service representative and interim lead when DNS was on vacation. Additionally I assisted with activities of daily living including assisting with treatments, incontinent care, catheter care, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, EKG. I performed various tasks assigned by the charge nurse including vital signs, resident’s weights, applying creams/ointments, collecting specimens, as well as lifting, moving, and transporting patients, using proper body mechanics or lifting devices.I ensured that individualized care plans were followed and provided the charge nurse or other care planning staff with specific information, changes and observations. Answer patient call lights; determine nature of problem; respond within scope and/or obtain appropriate resources. Provide support and assistance to residents/patients, which included those with Alzheimer’s and Dementia. Received phlebotomy certificate - Health Care Certificate “A” Responsible for assembling new and maintaining patient charts, current forms and results/reports and working with outside providers to ensure documentation is complete.Initiate and prioritize the accurate processing and transcription of provider orders. Responsible for verification of MAR and Service Plans monthly.Verify and ensure all audit material ready for biannual review by regional nursing supervisor. Communicate and interact effectively and tactfully with residents/patients, visitors, families, peers, and supervisors.Provide hands-on training for new employees in medication assistance.Responsible for staff scheduling and duty rosters for department.Pharmacy liaison for ordering, daily deliveries, and monthly cycle-fill of medications.Assess, order, and maintain unit supplies and equipment
