Provide leadership and direction to staff regarding daily workflow in this long-term care facility.Review all prescriptions entered into patient profile for appropriate therapy and accuracy as part of PV1 / Pharmacist Review. • Monitor profile for drug / disease / food interactions to reduce adverse effects. • Consult with MD for any adverse events identifying optimal solutions and providing top-quality care.• Interact with adjudication for accuracy of insurance payment to… Show more Provide leadership and direction to staff regarding daily workflow in this long-term care facility.Review all prescriptions entered into patient profile for appropriate therapy and accuracy as part of PV1 / Pharmacist Review. • Monitor profile for drug / disease / food interactions to reduce adverse effects. • Consult with MD for any adverse events identifying optimal solutions and providing top-quality care.• Interact with adjudication for accuracy of insurance payment to maximize revenue. • Abide by state and federal narcotic regulations to ensure adherence. • Implement formulary management and therapeutic interchange to optimize efficiency.Conduct a PV2 / final physical review of end prescription and product for accuracy, filled with the correct medication, correct quantity, correct directions, and for the intended patient with the correct prescribing physician.• Maintain and operate ATC machine for “pillow packing” medications to ensure medication accuracy.Manage IV infusion therapy - antibiotic therapy, TPN formulation, hydration, hospice IV therapy, CADD pump IV therapy, PCA IV pain management, IV iron therapy, hazardous IV drug therapy, chemotherapy, and fluid overload therapy.• Monitor pharmacokinetics via laboratory values - aminoglycosides, electrolytes, kidney function, fluid overload / dehydration, weight, vancomycin level to maximize positive patient outcomes and minimize adverse drug events.Make recommendations to MD and nursing staff to maximize resident IV therapy through pharmacokinetic monitoring.• Assess drug-drug interactions, calculate, and manage TPN and PPN formulations.• Oversee compounding of IV therapies and maintain sterility of compounded IV therapies. • Manage IV technicians to ensure proper compounding techniques and sterility.
