I set up and implemented a COVID-19 vaccination site. Specifically, this involved:-Writing guidance on the Standard Operating Procedures regarding the vaccinations.-Providing training to fellow clinicians on the COVID-19 vaccination.-Running local community COVID-19 vaccination clinics to increase uptake in the BAME community.-Dealing with clinical queries and incidents and writing Significant Event Analysis reports.I also participate in strategy meetings with the CCG.I run various clinics including minor ailments and medication review clinics where patients’ medications are optimised to ensure the best outcome for them, utilising a patient centred approach.I help the surgeries reach their QIPP targets and Universal Patient Offer.I am the Lead Pharmacist for our “Central Prescription Service”, which involves reviewing repeat medication and ensuring any relevant changes are made before signing the prescription off.Providing training for the various services run by the Federation such as the “Docman Service and Central Prescription Service”.Taking part and providing support on the Federation Respiratory and Diabetes MDT service.Completing surgery tasks, which involve me helping out with patient clinical queries, medicine reconciliation, and clinical MDT meetings and delivering training sessions.Disseminating information from the PCN DES Contract and implementing projects to help deliver the KPIs.Managing a team of Pharmacists and reviewing their performance on an ongoing basis through monthly “1-2-1s” to help with their development and align our goals for the betterment of the team.

● During my time at Boots, I completed my pre-registration training and held two store basedroles in Birmingham. One being in a health centre and the other being a large multi-servicescentre.● One of my highlights at Boots was the implementation and rollout of a project led locallyinitially and then nationally. I identified an opportunity for community engagement as well asincreasing the uptake of the Meningitis B vaccine to ensure more children were protected aswell as contributing to Boots’ strategic goals. By reaching out to local schools to raiseawareness, the success of this project resulted in it being implemented on a national scale.As a result of this, I was awarded “Pharmacist of the Year” at the Boots awards for the localarea.● I ran numerous clinics whilst at Boots, such as: Hair Retention, Meningitis B Vaccinations,HPV Vaccinations, Travel Vaccinations and many more. These clinics have taught me how tomanage clinics and the rest of the pharmacy’s work in a high pressure environment.● As a “Healthy Living Pharmacy”, we ensured that the local population was in good health andas part of this, I have been involved in promoting local and national Public Health Englandcampaigns with the aim to benefit the local community.● I have worked alongside various Care Homes to ensure patients have the best treatmentregimen possible. This was done by training carers and nurses to increase the quality of theirhealthcare to vulnerable patients.● I learnt how to work efficiently in a fast-paced environment, managing the patient volumeand prioritising patients depending on their needs.● I have conducted regular audits to improve patient care and medicine waste within thepharmacy.

● Completed Level 3 medication reviews on new patients and discharged patients within 5days of them coming into the home, leading to improved quality of life for the patient.● Created processes to measure and track KPIs to enable optimisation and reduce overheads.● Worked with the Pharmacy technician to help establish a plan to reduce medication waste.● Liaised with care homes to improve the working relationship and produce a seamless servicefor the patients.● I also monitored and provided training to care home staff in medicines administration andmedicines management and ensure competence in these areas prior to dealing withmedications.● Submitted monthly KPI spreadsheets as part of the MOCH project to assess the projectaccompanied with meetings with key stakeholders to evaluate the progress so far and anyareas which require further research.● Developed and maintained relationships with the local multidisciplinary teams to ensurepatients receive the best healthcare.● Worked alongside the local CCG medicines optimisation team to lead the MOCH projectsuccessfully.
