I worked on the nurse bank but worked all my shifts in A&E. I regularly worked in Ambulance Assessment, Resus, Majors, Minors, Ambulatory and on occasions also worked in the Clinical Decisions Unit and Paediatric area. I undertook all duties of a nurse working in these environments including, personal care, plastering, wound dressing, I/O and cannula insertion, trachy suctioning, arterial line set up, male catheterisation, and medication administration. I have also on rare occasions coordinated minors and ambulatory, a role usually conducted by band 6 or higher. The department is part of the major trauma network and I am frequently part of the trauma team where my role was primarily the administration of blood products during major haemorrhage cases. In non- catastrophic haemorrhage cases my role was flexible, and I undertook any task required that was with in my competency.

I worked late notice shifts in many A&E departments in the West Midlands, Northwest, and Yorkshire. I am allocated to an area of the department on arrival and have worked: triage, minors, majors, rapid assessment, and resus. I undertook all duties of a nurse working in these environments including, personal care, plastering, wound dressing, I/O, and cannula insertion, tracheostomy suctioning, arterial line set up, male catheterisation, and medication administration. At trusts that have no dedicated paediatrics nurses I have also worked in the paediatric A&E area.

I worked night shifts for staff bank in the A&E department.I worked throughout all areas of the department. Of particular note, in minors I was responsible for completing all treatments prescribed by ENP's and doctors including, plastering, suturing, and wound dressing. The department is part of the major trauma network, and I was frequently part of the advanced trauma team where my primary role would be for administering blood. In routine trauma and other Resus cases I was required to undertake any task within my scope of practice including I/O and cannula insertion, tracheostomy suctioning, arterial line set up and male catheterisation. In Emergency assessment I took a history and cannulated the patient if required and discussed the paint with a doctor if I felt they needed immediate medication or imaging. Standard nursing duties were also a key part of my role within all areas including medication administration, vital sign observations, safeguarding, and personal care.

My duties and responsibilities were to provide and facilitate all aspects of patient care whilst the patient is in the emergency department and to ensure that patients under my care are assessed and receive treatment within an appropriate amount of time. The care provided includes but is not limited to ensuring that patients receive appropriate food and drink, their personal hygiene requirements are met and trying to meet their emotional needs, i.e., keeping the patient informed and updated as to what is happening while they are in the department. The assessments and treatment that I undertook included initial patient assessment on arrival to the emergency department, continuing assessment using clinical judgement and vital signs to detect early patient deterioration and escalating any concerns to the nurse in charge/doctor managing the patient as necessary and administering prescribed medications. When appropriate I was required to delegate some of the assessments, treatment and care to junior members of the team. It is my responsibility to ensure the staff undertaking these tasks are competent to do so. Other duties and responsibilities include initiating safeguarding for vulnerable adults and children where appropriate, keeping accurate documentation, pro-activeness and communication with the multi-disciplinary team to prevent patients breaching the 4 hour time limit, preforming clinical skills such as, phlebotomy, cannulation and inserting both female and male catheters and wound dressings and applying plaster casts. The areas of the department that I was required to work in were; Majors, Ambulatory Majors, Paediatrics, The Resuscitation Room and the Trauma Room.

I worked for the Royal Surrey Staff Bank, however I booked the majority of my shifts directly through A&E. When working in the A&E department my duties and responsibilities were varied and included day to day cleaning of trolleys and equipment, restocking of the fluid cupboard, patient personal care which incorporates washing, feeding and toileting, nursing assessments, standard observations including lying and standing blood pressures, ECG's, peek flows and neurological. I also regularly cleaned and dressed wounds, applied Plaster of Paris casts and ran bloods through the departments FBC, VBG/ABG, D-Dimer and trop machines. When the resuscitation room gets busy, I was often asked to assist there. This involved caring for and monitoring patients in potentially life-threatening situations. I was required to work closely with other members of a large multi-disciplinary team including occupational therapists, mental health, adult and pediatric nurses, health care assistants and doctors. It was my responsibility to ensure pertinent information regarding a patient is passed to the team at an appropriate time and to assist with any other tasks required mentioned to allow the patient to receive the best care and treatment possible. On the occasions I worked on the wards my roles included personal care, washing, feeding and toileting, making beds, serving patient meals, observation rounds including blood sugars and generally helping patients with activities of daily living. I was required to work closely within the team of other healthcare assistants to ensure this all happened in appropriate time scales for the patients. It was also important to work closely with the nurse looking after the same groups of patients as myself so I can pass on and receive pertinent information to facilitate good patient care.

My duties and responsibilities changed a lot over the 7 years I served in the Army. I could be instructed to carry out any task, with little notice, requiring me to be dynamic and adaptive to the situation and surroundings, both while serving in the U.K. and abroad. While serving on my tours of duty in Afghanistan my role as an Infanteer meant that I was trained to be in front line situations, I was required to work under pressure as part of a team in physically and environmentally hostile situations. This role was not however limited to engaging with the enemy. I was regularly required to show compassion and empathy to local national civilians. I specialised in communications, and it was my job to ensure that the infrastructure to pass information within a company group was available in a timely, efficient, and effective manner 24/7. I was able to achieve this by using my initiative to improvise and adapt to an ever-changing environment.
