About Simon Meadowcroft

I have worked in the learning disability field for over 18 years and have been a qualified learning disability nurse since 2004. Since qualifying I have worked in an learning disability assessment and treatment unit, tier 4 CAMHS unit, 2 community learning disability teams, a specialist residential service and am currently the mental health liaison nurse for learning disabilities covering North Wales. I have completed an additional diploma in positive behavioural support. I have also completed modules in Forensics in Learning Disabilities, Person Centred Counselling and Research Methods to gain my BSc Undergraduate Degree.I am currently working towards MSc in Leading Quality Improvement in Health Care at Bangor University. 2016 has been a great year with the Mental Health Act leaflets I designed being highly commended by the British Medical Association before being adopted by the Welsh Government to compliment their Revised Mental Health Act Code of Practice (2016) and will be on their website within the next month. This has led me to be award the advancing equality award for BCUHB.

Mental Health Liaison Nurse For Learning Disabilities

This exciting new position has given me a lot of autonomy to develop this post covering North Wales. I am heavily involved with crisis prevention and support the majority acute in-patient admissions for people with learning disabilities and co-morbid mental health problems across 4 hospital sites. I ensure that reasonable adjustments to environment and clinical practice are implemented and developed a number of augmentative documents for this client group. A core element of this role is promoting partnerships between mental health and learning disability services to deliver and plan good quality, joined up services and support for this client group. I deliver mental health and learning disability training for CLDTs and the police and have written a Dual Diagnosis acute admission policy which is currently going through ratification. I also assess service user's mental health in general hospitals, provide information and advice for mental health services across North Wales and actively promote the use of evidence based assessments and treatments both in the community and in-patient settings.

Community Learning Disability Nurse

This role involved having between 15 to 20 service users on my case load with complex health and or behavioural needs. I utilised a range of assessment tools to implement a range of effective, evidenced based behavioural and clinical interventions. I played an active role in a number of service developments such as leading the development of the Behaviour Advisory Sessions (BAS, please see attached documentation) which has played a major part in significantly reducing the community nurse waiting list. The BAS model is now hoped to be adopted in other community teams across the university health board.I provided primary health interventions through groups and on a 1 to 1 basis as well as co-ordinating and completing a number of joint and CHC funding applications, which required a good understanding of the process and continuing health care criteria. I also co-ordinated and completed clinical reviews identifying saving, streamlining support where possible and ensuring that services providing progressive care and treatment.I worked as clinical lead for transition of the 4 years working with young people extremely complex needs, as well as working closely with forensic services often involving in MAPPA level 3 and CPA review in prisons and medium secure units. I regularly took an active role in the safegarding process requiring a good working knowledge of the requirements and procedures involved.I developed and deliver a presentation teaching pack for first and third year general nursing students and mentored some return to work students and supervised another nurse.
