Through screening I identify potential microvascular and macrovascular complications of diabetes and perform the relevant surveillance e.g., chronic kidney disease providing holistic care andidentifying the most appropriate therapies to manage diabetes and the associated risk factors for that individual. Insulin starts and insulin management also being part of my role.I have developed good relationships with secondary care for the benefit of all reducing hospital admissions and promoting the smooth transition of patients across services.
