• Work with Council project coordinator to ensure all community based supported priority areas are captured in annual and quarterly work plans particularly continuity of treatment plans and budget to support the implementations.• To ensure adequate integration of facilities and community based activities that will increase identification, enrolment and retention in provided services (HTS, PMTCT/EID, ART, TB and family planning).• Work with facility and community actors (CSO/FBO, support groups and CHW) to coordinate facility community based treatment supportive activities that increase access demand of health services provided at health facility and community level such as ART, PMTCT, Family planning, adolescent and youth friendly services.• Guide implementation being done by community CBHS and CHWs services focal person at regional, district and facility levels in collaboration with coordinators (RCBHSco, CBHSco and supervisors) on planning, implementing and reporting of the community engagement activities ensuring increased awareness, HIV testing, referral, enrolment and retention.• In collaboration with CHMT, engage community actors to support health facilities to improve community based treatment supportive services to improve treatment adherence and clinic attendance.• Provide TA to health facility staffs and CHW to strengthen paper based management referral’s system which include proper filling of referral forms, monitoring of returned referral forms and keeping records appropriately.• Coordinate and ensure planned training and workshops or meetings for community stakeholders (lay counselors, CHW, Treatment adherence counselors, CBHS coordinators/supervisors, local leaders and CSOs) in the project are well coordinated and implemented in high quality and follow up is made for expected output or outcome.• Build capacity of facility staff and CHW to improve treatment adherence and PSS counselling and provision of PHDP package and retention on services.

• Site support to facilities and TA provision to ensure LCM (Linkage case management, tracing of missed visits, and lost to follow up as well as supporting GBV/VAC and TB contact tracing by using CBHS/CHWs and EX-TB groups members and volunteers.• To conduct on job trainings for community volunteers in the project areas like HIV/AIDS, community family planning, TB, GBV and coordinate volunteer’s efforts.• Capacity building to facility staffs on various retention strategies (FLAGs and three boxes model.) to create a friendly environment to clients and improved adherence.• To prepare budgets to support the implementation of CE activities at regional level (IIT tracing, supportive supervision).• Work with RAS (Regional Accelerated Strategy) team on supporting facilities on the improvement of poor performing indicators and ensuring improvement by providing TA on the gaps.• To compile monthly and quarterly programmatic reports.

• Formation of Facility Linked Adherence Groups (FLAGs) and linkage of clients to groups for income generation activities and treatment supports.• Actively guide and follow up patients for continuity of services in PMTCT, EID, PITC, C&T, TB and STI units including the review for accuracy in service documentation in respective registers in weekly basis.• Supervising the provision of gender and youth friendly services at the facility and school clubs.• Organizing and ensure the provision of male friendly services at the facility in HIV case identification, enrollment and retention in care and treatment.• Prepare a list of PLHIV and HIV exposed children who have missed an appointment and/or are lost to follow up, on a weekly basis.• Actively trace HIV exposed infants and PLHIV who are lost from follow up on a weekly basis, through Community Based HIV Providers (CBHS), Community Based Organizations (CBO) or PLHIV support groups and provide feedback on the outcome to the right services.• Establish PLHIV groups, including for children, and actively collaborate with these groups to increase psychosocial support, to reduce stigma ad to decrease loss from follow up.• Register, strengthen and collaborate with CBHS volunteers in their HIV related activities.• Builds capacity and coordinate roles for expert clients in HIV case identification, linkage, treatment and retention in care• Encourage PLHIV groups in taking active roles as peer educators during group counseling sessions.• I championed to develop a service provision protocol for PLHIV clients directory and referral system between departments within the facility and outside the facility. • Prepare monthly reports on community related activities and submit to EGPAF.

• Advocating rights of marginalized groups like elders, people with disabilities, most vulnerable Children’s and gender based violence.• Conducting and facilitating customer care trainings to health service providers.• To pay home visits to patients with viewed barriers to treatment for the aim of establishing factors set-backing their treatment.• To arrange clients’ group therapy in different departments, such that pediatrics, CTC works, community works, maternal and child health as well as gynecological social problems. • To carryout rehabilitation services for mental & physical handicapped and general casework including staff welfare services.• Reporting, documentations and sharing of best practices.• To conduct health education to patient & their families. • To highlight social factors relevant to the origin of the disease.
