Clinical terminology plays a mission-critical role in building integrated Healthcare Knowledge platforms & applications. Towards this, I headed a Terminology services initiative building a brand-new enterprise-wide Clinical Terminology capability from ground up to operationalize ontologies (SNOMED, LOINC, RxNorm, ICD, CPT, etc.), value sets & cross-maps using Semantic Web frameworks and providing a FHIR API & tools for use in CDS, claims processing and other enterprise needs. Developed the long-term product roadmap and architecture, established governance model for terminology assets, provided ontology training and grew the team to 18 members. Optum is developing a Clinical Decision Support (CDS) platform for integrating with EHRs and providing point-of-care decision support. Serving as an expert to incorporate Real World clinical needs for "CDS" and develop a strategy for creating a Clinical Knowledge Engineering Lifecycle Environment and Knowledge Delivery Platform, adopt open data standards and CPG-on-FHIR (HL7 CPG-IG STU v1.0) specification, develop Guideline authoring process, and the use of terminologies.

The COVID-19 Digital Guideline WG is using an agile approach to develop Clinical Process Guidelines(CPG) to aid the management of patients in the ongoing pandemic. Ontology development and use of standard terminologies is a core part of the agile process to incrementally, and rapidly develop the clinical, knowledge engineering and execution aspects in parallel using FHIR, CQL and BPMN frameworks.

The Knowledge Enriched Data (KED) project aims at integrating data from multiple systems with semantic enrichment to support discovery, clinical workflows, decision making and research. Serve as Ontology Expert on overall application of Ontologies and related Knowledge Engineering Strategy for initiatives in the Applied Clinical Informatics Program at Mayo Clinic. Lead the Work Domain Ontology (WDO) development effort, enable Semantic Mark-up (Annotation), ontology support for Information models, and serve as a Biomedical Informatics and Knowledge Discovery expert.

Knowledge Content Management System (KCMS) leverages Mayo’s knowledge and expertise to support teams that generate content for consumers, patients, and providers. As a member of the external review board, interviewed stakeholders and examined data needs, current architecture, delivery pipeline, etc. and provided recommendations on a knowledge management strategy including knowledge architecture, content models & curation, terminology/ontology, and best practices for a changing environment.

Scientific advisor for the Newborn Screening Follow-up and Translational Research Data Integration Collaborative (NBSDC) and collaborator on the Ontology for Newborn Screening Follow-up and Translational Research (ONSTR).

Cross-functional clinical informatics leadership for emerging technologies and interoperability needs using semantic web architectures and clinical terminologies including SNOMED, RxNORM, MeSH, ICD 9/10, CPT and LOINC. Headed clinical terminology services responsible for development of Elsevier Merged Medical Taxonomy (EMMeT) and its application across multiple products such as ClinicalKey, CPOE, Clinical Decision Support and Linked Data.

Visiting Faculty focussing on research into using Natural Language Processing (NLP) techniques and clinical models such as UMLS and SNOMED-CT for information extraction from clinical narrative text. A significant portion of the clinical information in the EMR system is in the form of expressive narrative text that is outside the scope of analytical tools. Information extracted from the text is important for data mining for quality improvement and patient safety.

Lead a NIH/NCRR funded research project across four CTSA institutions (CWRU/UH, Marshfield Clinic, University of Michigan Medical Center, University of Wisconsin Medical Center) developing a federated data integration system supporting clinical and translational research. Trained cross-functional teams in using Agile methodology for clinical and translational informatics research. Chaired the technical design and architecture sessions across the four organizations working on the PhysioMIMI system. Developed a Sleep Domain Ontology - an applied clinical domain model with cross-maps to ICD-9, ICSD, RxNORM and SNOMED for Sleep Medicine.

Heart and Vascular Institute's SemanticDB project is a pioneering research effort harnessing Semantic Web technologies to integrate data from multiple clinical data stores for clinical research and outcomes studies. Introduced Agile methodology in coordinating the first production release of the project in 2008. Authored a Core Data Elements model for Cardiology and Cardiothoracic Surgery with integration to standardized terminologies such as SNOMED-CT for ACC and STS quality measures and outcomes reporting.

NMS developed a highly customised and user friendly EMR system. However, maintaining the customized Clinical Content solutions for clients was challenging and needed a reusable and scalable solution. I lead the re-architecture and development of a Standard Clinical Content Core data model resulting in ease of configuration and customization with a 60% saving in implementation costs for new clients and improved the Time to Market by over 50%. Implemented reporting enhancements and utilized SNOMED CT and other clinical models for clinical knowledge development and integration. Built up the Medical Knowledge development team and trained developers in both the clinical and technical aspects of content development.
