• Integrate an independent retail pharmacy in a newly developed medical plaza Sugar Land, TX• Provide medications and compounded material to patients • Execute day to day Pharmacist duties related to prescription based medication delivery• Engage in regular communication with Physicians in medical building regarding DUR, drug interactions, side effects, pharmacokinetics/ pharmacodynamics, and collaborative practice• Interpret physician orders for home health infusion patients• Provide home health infusion medications/specialty medications in accordance with ACHC guidelines• Compound medication for home health infusion patients using aseptic technique• Perform clinical follow up on infusion patients based on labs, symptoms, and other clinical markers

• Act as a per diem pharmacist for a mail order specialty pharmacy • Provide medications that are not readily available to patients • Execute day to day Pharmacist duties related to prescription based medication delivery• Engage in regular communication with Physicians in medical building regarding DUR, drug interactions, side effects, pharmacokinetics/ pharmacodynamics, and collaborative practice• Perform clinical follow up on patients needing extensive monitoring based on labs and physician orders
