Wraparound Facilitator • Coordinate and facilitate Wraparound team meetings. • Provide leadership in the development, implementation and evaluation of individualized plans using the phases and activities of Wraparound.• Collaborate with teams to identify needs and create a shared vision and mission for the family. • Assist families in identifying, recruiting and engaging natural supports.• Support clients in identifying their strengths and building upon them.• Develop the problem-solving skills and self-efficacy of the youth and team members.• Identify barriers to full participation in the Wraparound process and develops strategies to overcome them. • Consult and advocate with CSW, probation, and DMH liaisons on the family’s behalf to ensure quality services are being provided.• Ensure the Wraparound processes is completed with high fidelity to the model and tracks progress toward goalsMental Health Therapist• Provide individual and family therapeutic service to assigned clients under the Mental Health Program.• Ensure that services are driven by the client’s needs and desires to empower the client to take charge of his/her life with informed decision making.• Develop the Coordination Plan/ Plan of Care/ Safety Plan with the child/adolescent and family (and other support persons/agencies) when applicable.• Consistently assess the individuals progress toward meeting the Personal Milestones outlined on the Service Plan.• Monitors, supports and assists the client in developing or maintaining skills needed to achieve the individual service plan including medical, dental and psychiatric services; guidance in money management, and the use of education, socialization, rehabilitation and other social services.• Interacts on behalf of the client with family, friends, legal guardian, conservator & significant others to initiate and maintain supportive networks.• Performs evaluations/assessments to determine medical necessity & coordinates the treatment plan.
