This is a support service for stroke survivors and their families and carers in Hammersmith & Fulham who have been affected by stroke. The service aims to be involved as soon as possible after a stroke, to give information, advice, case work support and emotional support, thereby optimising quality of life and life opportunities, and enhancing the ability to cope with the practical and emotional issues of disability. In cooperation with volunteers as relevant, the service also aims to offer basic befriending to service users, hold drop-in advice sessions, give stroke-related presentations, including primary and secondary stroke prevention, and hold or take part in relevant events in the borough.
