As the Minnesota Territory Manager, I was responsible for hiring, training and directing the work of our Team Leads, Team Coordinators, and Medical Team Assistants. I collaborated with the Market Director, Clinical Director and Clinical Field Manager to ensure the maintenance and growth of all 10 provider practices serving our geriatric and disability population in the Minnesota market. Other key responsibilities included: 1.) Management of all team schedules, including community on-boarding & community transitions, coordination of PTO coverage, and management of the provider float pool, 2.) Revenue Cycle Management including oversight on billing, coding, receivables management and the communication & implementation of new policies & procedures for the Minnesota market, 3.) Oversight of 120+ communities in the Minnesota market as well as related service partners in those communities (e.g., Home Care, Hospice), and 4.) Marketing to new families and patients in the service communities to fill team panels as well as assisting the Market Director with acquisition of new communities in the Minnesota market.

Key responsibilities included: Coordination of scheduling for 500+ patients in 15 communities; Engagement with a diverse group of external customers including community nursing staff, home care & hospice partners, patients and families; Ownership of the team schedule; Oversight on team billing, coding and performance metrics and; Management of all internal & external team communication via phone, email, EHR and secure online communication portal.
