Currently I provide technical assistence to existing programs while helping the church develop current programs that will enable the church to strengthen its community position. The most recent initiative was coordinating the recent Clean and Green clean-up event in both the Auburn-Gresham and West Chatham neighborhoods.

As Senior Case Manager I maintained a caseload of sixty (60) families who resided in the Cabrini Green Housing Project. I provided intake and assessment to each family member who was eighteen (18) years of age or older; each individual received an Individual Action Plan,if followed, allowed the family to remain lease compliant. My responsibilities were to ensure that the head of household was gainfully employed or engaged with our employment program.All individuals who were eighteen (18) years of age or older who were not in school were also referred to our employment program. Individuals who were not eighteen (18) years of age were required to be enrolled into an afterschool or park district program and when appropriate to the Mayors Summer Work Program. All participants received monthly home visits and sixty (60) day evaluations.The Senior Case Manager was also responsible for creating individual files and concise case notes. The Senior Case Manager was also responsible for weekly, monthly and quarterly, statistical reports which were entered into our Salesforce data entry system.

Recruited fifty-five (55) families who resided in the former Cabrini Green Housing Development. The Head of Household of each family was intaked and assessed; each family received an Individual Family Action Plan, tailored to assist each family in achieving the goal of self-sufficiency. This was achieved by assisting a family remove any obstacles that prevented self-sufficiency progression e.g., employment/training, adequate child care referrals, school reinstatement, referrals for substance abuse treatment, housing advocacy and rental assistance. I was also responsible for maintaining individual case files.Conducted monthly face-to-face contacts; submitted weekly,monthly and quarterly reports per program requirements.Case Manager maintained a ninety-seven (97) percent engagement ratio.
