•Responsible for coordinating services for Severely Emotionally Disturbed (SED), Medically Fragile and Developmentally Delayed youth that are outlined in each child or adolescent’s Individualized Health Plan (IHP). •Assess the need for services, develop and update the IHP, connect the child and family/caregiver to identified services, consult with the child and caregiver, and to advocate for the child’s needs.•Responsible for monitoring the Waiver Service Provider’s (WSPs) work in relation to the requirements of the goals outlined in IHP.•Link between the child and the Waiver Service Provider and conduct team meetings to discuss services and facilitate communication between caregiver and Waiver Service Providers. •Communicate with the child’s caregiver/family to construct the plan and arranges for the necessary medical, social, rehabilitative, vocational, and educational services that the child needs. •Twice a month travel to have contact with the child and caregiver/family in the home and the community.

•Developed and sustained ongoing working relationships with families through regularly scheduled home, school, service provider and office visits and interviews to accomplish stability and permanency. •Maintained a complete case record for each client, who documents progress through admission, under care and discharge: prepared and submits dictation, memoranda, referrals and reported on each case on a timely basis. •Ensured full and timely written and verbal communication with the New York City Administration for Children Services and the Office of Children and Family Services on all case aspects and developments.•Represented Graham-Windham and testified if required, in any court hearing related to caseload.•Participated in staff meetings, seminar and trainings in order to maintain and apply a current knowledge of internal and external regulations and policies. •Responded to client emergencies within or outside work hours.

•Completed field visits in order to investigate allegations of child abuse/neglect in accordance with City and State protocol.•Interviewed family members and collaterals to assess child safety. •Removed children into protective custody or foster care, when suitable. •Entered and sustained accurate and timely computerized records of all case contacts into state database. •Filed petitions and testified in family court and other legal proceedings.
