Intraoperative monitoring is the monitoring of the CNS including me brain, spinal cord and nervous system during surgery. I help prevent injury, this is accomplished by sending an electrical signal up and down the spinal cord to establish a baseline. Once baseline is established it is my responsibility to provide surgeon with live verbal feedback of any changes during the surgery! I set up the machine and run monitoring pods and stimulation sources neatly across the operating bed.After the patient is asleep and the nurse has placed the Foley, I will Place small EMG needles and electrodes in the appropriate muscles. Once all electrodes are placed on the patient they are then connected to the cascade cadwell equipment and the baselines are established the typical baseline for most surgeries consist of motor evoked potentials (MEP) in somatosensory evoked potential (SSEPs) and electromyography (EMG). This is what I see and report to the surgeon before incision.While the case goes on I will keep the surgeon informed of any changes in the baseline, which means the surgeon is causing nerve root irritation during that portion of the surgery.After the case is finished the surgeon will be notified of the baseline remaining the same which will make the surgeon and most importantly the patient very happy to be able to move and feel all extremities! :)
