Responsible for a caseload of 64 residents including 24 TCU and 40 LTC residents Ensure care continuity for discharges from facility to communityFacilitate monthly resident councilInvolved in activity program, safety committee, diversity committee, as well as daily department head meetingsComplete all SS functions in regards to MDS and care planning

Facilitate the social and psychological well-being of residents and their families through the development of individualized resident care plansPlan and ensure appropriateness and care continuity for transfers within and discharges from the facilityInvolve the entire facility in meeting residents' psychosocial needsPerform social service admissions functions

Member of marketing team for the memory care units Facilitated monthly Alzheimer support group (Trained by Alzheimer Association)Provide direct casework services for a caseload of 80 residents and their families Petitioned for guardianship and conservatorship services for residents when needed Identify social/psychosocial needs of the residents

Plan, implement treatment, and transition based on psychosocial assessment that is integrated into interdisciplinary care planPracticed a range of social work interventions that included casework, counseling, crisis intervention, case management, and group work methods in assisting patients and families to reach their goalsArranged, mobilized, utilized, and coordinated a comprehensive range of community resources to most effectively meet the identified patient and family needsFormally and informally educated patient/family and health care team members regarding the social work role and pertinent patient care issues
