Social Worker- ECCO ServiceProviding an enhanced model of care to the Drug and Alcohol Service to ensure a holistic and coordinated approach to complex clients. This includes the provision of consultation and liaison to clinicians, presenting at various education seminars within the South Eastern Sydney Local Health District, undertaking complex referrals including disability services (ADHC, NDIA), Aged Care services, mental health community case management and referrals for Involuntary Drug and Alcohol Treatment. This role specialises in community outreach and home assessment, as well as care coordination and increasing external service accessibility for the most marginalised patients with multifaceted and complex needs.

Social Worker- IMPACT ProjectReducing the rate of preventable ED presentations for people with chronic substance use disorder with multiple complex co-morbidity who present frequently, through the provision of assertive outreach, care coordination, intensive case management and creation of pathways to appropriate services.

Case Manager- ICIS Homeless Project & Common Ground Project Key Tasks Included: • Assess and respond to referrals to program• Advocate for clients to receive priority housing assistance • Assist clients to access emergency, temporary and respite accommodation as needed• Support clients to transition to housing and develop independent living skills • Liaise regularly and build relationships with local government and non-government services; developing networks and referral pathways• Develop individual case plans and coordinate care to address needs of clients and facilitate access to relevant programs and services• Crisis intervention with clients who exhibit complex and competing needs• Assist clients to maintain tenancies; advocate on their behalf• Assess needs of client and make applications for Guardianship/Financial Management and Involuntary Treatment as necessary
