Helping people is what we do!• Performs all care coordination, disease management, and case management functions telephonically or in-person, as appropriate. Assesses patients’ needs and establishes goals for each case. Reassesses patient’s status and reviews plan of care on open cases at scheduled intervals. Documents activities correctly and communicates to appropriate resources.• Works with patients, and their families, to develop a plan of care in consultation with other disciplines, social work, insurers, and health care team, as necessary. Counsels patient to consider care solutions representing best value in terms of both quality and cost of services being considered.• Recognizes, supports, develops, and mobilizes informal and formal resources to meet patients’ needs.• Arranges for needs services and entitlements, working cooperatively with patient, family and service providers. • Inputs necessary data and maintains automated software programs for patient tracking and organizational record keeping.• Provides emotional support through relationships with patients and caregivers.• Monitor patient satisfaction to ensure quality of services provided.• Takes ownership for the completion of each Quarterly Summary report.• Works collaboratively with leadership to review Program data results and provide feedback in a timely manner.
