I was asked by Senior Management to integrate the Health Care Home Program at the Whittier clinic and was promoted from the Ambulatory Clinical Training Coordinator to the Clinical Care Coordinator for Health Care Homes.-Collaborated with faculty, residents and mid-level team of multi-specialty site with 50+ providers (FP, OB-GYN, Psych, Same Day Surgery, Alternative Medicine)-Utilized and collaborated with community resources (contracted services, hospital navigators, in-patient care coordinators, HSPHD services/programs) to ensure the patient is receiving the necessary care throughout the individualized continuum of care-Worked with MHP/Hennepin Health team to decrease the number of emergency department visits and readmissions of patients-Care planning, follow up and interventions for the patient with visits to their home, hospital, clinic or other service location-Utilized EPIC dashboard for inpatient stay reports, discharge target reports, no-show reports with documentation and chart review via EPIC-Attended Complex Case Study class for residents and presented cases for review and input-Facilitated daily huddles for interdisciplinary team of SW’s, Community Health Workers, Clinical Coordinators and Legal Aid-Involved in process improvement principles as determined by senior management. Direct the day to day operations of the multi-specialty clinics, pediatric clinic, ob-gyn clinic and internal medicine clinic. Responsible for managing approximately 75 staff and worked directly with the providers and mid-level providers.-Coordinated financial reporting as well as problem solving, communication, quality and performance improvement of patient satisfaction and employee engagement scores.-Managed 75 professional and para-professional staff including coaching, mentoring, hiring, firing and developing performance improvement plans in collaboration with HR and Union representatives.-Coordinated strategic initiatives and process improvements for the clinic with senior leadership.-Designed and implemented an internal float pool within the clinic to decrease the cost of staffing resources.-Coordinated and Implemented MN Community Health Initiatives with Behavioral Health, OB-GYN, PEDS and Internal Medicine.

Direct the day to day operations of the Blood and Marrow Clinic in collaboration with Fairview Health Systems and the U of M Academic department.-Developed, implemented direct patient-centered care delivery systems for the clinic including outpatient, inpatient lab units to insure continuity of care across the continuum.-Identified and lead performance improvement initiatives for customer service, patient access provider access and EMR implementation.-Supervised approximately 45-50 professional and para-professional staff with emphasis on employee engagement and accountability as well as customer service.-Practiced nursing in the clinic and directed nursing practiced in collaboration with the Ambulatory Director of Nursing.
