Provide information, support, and communication with case managers, guidance counselors, teachers, students, and families. Provide leadership on the district implementation team for Individualized Learning Plans. Work with the Guidance Department on transition initiatives and events for all students. Research post-secondary options and creates a collection of resources for students, parents and colleagues. Maintain and establish connections and community partnerships with outside agencies, colleges and service providers. Serves as a resource on post-secondary opportunities for all students and their families. Provides transitional support as students enter a new school. Evaluates transition materials, administers transition assessments, and analyzes results to create reports of these assessments. Track the delivery of each transition-related service for all students. Maintain accurate and confidential records of all assigned students. Ensure parents and service providers are informed of transition activities and options and invites them to participate with students as appropriate. Provide information specific to post-secondary transition requirements in accordance with state and federal regulations. Provide workshops, presentations or professional development for teachers, parents and students on topics related to post-secondary transition. Attend individual student transition and/or IEP meetings as required.

Special Education teacher who is the program coordinator for the 12+ Program at Attleboro High School. The 12+ Program is a transition based program for special need adults ages 18-22 years of age. The focus of the program is to provide these young adults with vocational and independent living skills, in order to prepare them for adult life when they age out of High School at age 22. I create and implement functional academic lesson plans, based on individual student need. I also create vocational and community experience opportunities, with partnerships I have created within the community, in order to provide these students with vocational and independence skills. I maintain a caseload of 9 students, ensure the implementation of their IEPs, communicate and collaborate with community partnerships, administration, service providers, parents and adult service providers. I ensure the smooth transition from high school to their future goals.

Teach US History and History electives to students in an alternative high school setting. The Attleboro Community Academy is an evening diploma program for students who may need an alternate setting or are taking an alternative path to earning their high school diploma. Created the curriculum for US History A, B, C, and D. As well as modified it to fit the schedule of the Attleboro Community Academy as the classes only meeting for an hour and half twice each week. Implement lessons, track attendance and progress. Communicate with administration to ensure a cohesive support system for each student to be succesful.

Sub-separate special education content teacher. Created and implemented modified lesson plans with specially designed instruction for special education students in the areas of Biology, Forensics, and varying levels of World and U.S. History. Maintained a caseload of 14 special education students, ensured the implementation of their Individualized Education Plans, conducted annual reviews, created and updated progress reports as related to IEP goals,communicated and collaborated with service providers, administration, special education colleagues and parents.
