The Behavioral Health Clinician (BHC) is a new role supporting an expanded integrated medical & behavioral health care delivery model. The BHC is a licensed, credentialed mental health provider with advanced skills, working as an embedded, core member of the health care team. The BHC role has five primary functions:1. Assessment: The BHC conducts assessment of behavioral health issues and conditions by means of diagnostic and screening tools, clinical interviews and consultation with the medical team as directed by the physician and in collaboration with the primary care team.2. Consultation: Provides consultation to the medical team aimed at the prevention, identification, treatment and management of behavioral health/psychosocial issues and conditions impacting overall health status of the defined patient population in the integrated care delivery setting3. Team Support: The BHC provides support to the team in managing the day to day behavioral health issues that arise in the clinic setting. The BHC promotes and facilitates a therapeutic environment within the clinic. The BHC also supports healthy and productive team work and team dynamics among clinic staff.4. Direct Care: The BHC provides therapeutic interventions addressing mental health and substance use disorders, as well as the behavioral/psychosocial aspects of routine and complex medical conditions in a collaborative and co-management approach with the health care providers. 5. Population Health: The BHC works with the care team to identify target populations and/or issues. They collaborate with the team to monitor and target support of these patients and families to address the identified issues – such as ED over-utilization, avoidable inpatient re-admissions, depression symptom reduction, moving toward specific health goals, etc. – in order to move toward the “Triple Aim” (improved patient experience, high quality care delivery, and reducing the total cost of care).

Supervises Behavioral Health Clinics (BHCs) across the MHealth Fairview system. This includes hiring, orienting, coaching, evaluating, and effectively managing performance to ensure delivery of high quality service to clients.Supervises and supports the day-to-day operations of the Integrated Behavioral Health program at MHealth Fairview. This includes promoting open and effective lines of communication, solving problems by anticipating program needs, formulating creative responses and strategies, initiating the needed interventions, and following through to resolution. Provides clinical consultation support to BHCs throughout the system, as needed. Helps lead BHC Case Conference meetings.Partners with Manager and/or Director to evaluate, develop, and revise clinical systems and to enhance clinical effectiveness as needed. Provides direct clinical BHC services at the Clinics & Surgery Center. Conducts diagnostic assessments, applies evidence-based clinical interventions, facilitates therapy and provides education for patients and their families.
