My true belief that no matter how seemingly insignificant all newly acquired "Knowledge Is Power". As such my primary objective is to "pay forward" my base of experience acquired throughout my 35+ year career, in part providing the knowledge to "Extinguish" the "Inextinguishable" fires. My formal education included extensive basic fundamental knowledge which have become extinct in most every phase of formal education. Although many this is one prominent example. After having reviewed the findings from a multitude of independent medical and scientific studies performed domestically here in the United States and internationally by many recognized as pioneers, scholars and experts in their respective research professions, i.e., oncology, vaccines, genetics, small and large molecule development, and the like. Upon completing my own comparative result research it was quite evident that the findings reported from a large majority of the case studies performed were very similar, not necessarily the resulting values in and of themselves, but one could say almost identical in quantitative scope. Cumulatively, the reported findings themselves make for a compelling argument strongly suggesting that these often times lethal fires were spawned direct by-product of what has become a "plug and play" laboratory operational mindset, with financial concerns such as ever tightening budgetary constraints, the most significant contributing factor in my humble opinion for the decline in consistently providing the highest quality of patient care, the palpable lowering of morale and once ever present feeling of "Team" healthcare professionals once shared, which I can define in 3 letters "IPO".

As East Florida Divisional Quality Assurance Coordinator, Clinical the scope of QA oversight responsibilities was essential the same as in my dual role as Regional QA Coordinator, AP/CP, differing only by primary focus being on the operational and testing proficiency of the Clinical Laboratory departments, i.e., Chemistry, Hematology, Blood Bank, Microbiology. As East Division QA Coordinator, oversight was limited to only those facilities functioning within the area designated as the East Florida Division, which consisted of approximately eighteen medical centers and their laboratory testing facilities.

As Regional Quality Assurance Coordinator, AP/CP laboratories I was tasked with the Quality Assurance oversight of all relative operational functions to include staff technical and non-technical relative to maintaining the highest degree of operating proficiency, technical performance, quality of results, sustained performance competency, and consistently meeting or exceeding expected task specific production levels at laboratory testing facilities operating within all HCA HealthCare, Inc. owned and operated 70+ medical centers and treatment facilities throughout the State of Florida. Collaborative investigation with facility specific laboratory management personnel to determine the cause, origin, and severity level of direct and/or potential for patient harm of identified Non-Conforming Event (NCE). If resulting in direct patient harm or a precalculated level for its potential stop-gap measures were immediately instituted and a comprehensively detailed and timeline specific Root Cause Analysis (RCA) investigation was performed. Existing preventative measures were collaboratively reassessed and revised accordingly, or when necessary, newly developed and implemented. Direct participation in scheduled and random QAP, Regulatory Affairs, and "mock" laboratory inspections for laboratory licensing and accreditation purposes.

My responsibilities were diverse in performance, scope, and objective focus. Although many in number my daily primary responsibilities focused on performing automated Prognostic Image Analysis of submitted malignant neoplasm cases and the Digital Slide Image Transmission of Histochemical, IHC, ISH, and FISH stained slides requested by distant facilities to minimize delays associated with the physical transportation and delivery. Daily focus was improvement on pre-analytical specimen preparation, staff proficiency in meeting expected production levels, and the consistent production results of the highest quality. I was also tasked with the daily microscopic review of 20% to 30% of completed cases from each submitting facility, either approving for distribution or rejecting the diagnostic quality requiring repeat testing until the diagnostic quality was acceptable.

Primary responsibilities included but were not limited to the gross dictation, dissection, preanalytical sample preparation of received surgical & non-gyn cytology specimens. When requested I was responsible for the stat Frozen Sectioning and rapid diagnostic histochemical staining of fresh tissue samples for rapid microscopic diagnosis by the Pathologist. Assisted in the obtaining of FNA biopsy specimens at patient bedside then performing rapid testing for immediate diagnosis and/or adequacy purposes. Responsible for all transcription functions, special procedure requests, periodic maintenance of existing and/or creation of new laboratory SOP manuals and regulatory required documentation pertaining to all aspects of laboratory operations, technical functions and other associated laboratory functions as required by licensing and accreditation governing bodies (i.e., JCAHO, CMS, CAP, CLIA, ASCP, OSHA, NCCLS)

Duties consisted of but were not limited to the overall technical and administrative oversight of all Anatomic Pathology and Cytology Laboratory functions and staff performance proficiency , to include manual and automated Immunohistochemistry, DNA Hybridization for HPV, implementation and operation of Ventana Benchmark automated IHC instrumentation and ChromaVision Automated Cellular Imaging System (ACIS). The laboratory staff under my supervision consisted of 20+ Histotechnologist’s, Histotechnician’s and Laboratory Aides on four (4) designated work shifts. Administrative duties included employee evaluations and development, competency testing, continuing education, departmental budgeting and capital equipment forecasts and expenditures. Technical duties included functioning as Education Instructor and Proctor of the on campus school of Histotechnology, providing direction to achieve daily workflow objectives, daily utilization and administration of the Cerner LIS, performing routine and complex histochemical stain procedures, IHC, HPV (High-Low Risk) ISH probe detection, interpretation of test results for QC purposes, and the as needed revision of existing or development of new SOP laboratory manuals when necessary as required by differing licensing and accreditation governing bodies, i.e., JCAHO, CLIA, ASCP, CMS, CAP, OSHA, DOH.

Responsibilities encompassed the overall technical direction and administrative support of all Histology Laboratory Operations to include Immunohistochemistry. The Histology Laboratory staff under my supervision consisted of 85+ Histotechnologist’s, Technicians, Laboratory Aides and Administrative Personnel on four (4) designated work shifts. Administrative duties included but were not limited to employee evaluations and development, competency testing, continuing education, departmental budgeting and capital equipment forecasts and expenditures. Technical duties included the overseeing of daily workflow, manual and automated Immunohistochemistry protocols, interpretation of test results for QC purposes and the implementation and/or revision of test procedures and related laboratory manuals as outlined and regulated by associated governing bodies (JCAHO, CAP and ASCP). With accreditation by the Hartford Community College School of Histotechnology, as designated Educational Administrator and Proctor I was primarily responsible for the implementation and providing formal class room instruction to students of an on-campus “School of Histotechnology”.

Responsibilities included but were not limited to utilizing a complete working knowledge of all Histology Instrumentation, Clinical Microscopes and ancillary supplies in conjunction with acquired Histology Technical and Application expertise in providing complete and in-depth product usage and applications support to end users and sales staff via various instructional environments. This would include workshops, seminars and product specific literature. Additional responsibilities included participation in new product design and development, product testing, issues of quality assurance in addition to the production of technical reports, product specific bulletins and newsletters. On site product installation and user in-services were performed as was product “familiarization” workshops for in-house staff. Utilized of a thorough working knowledge of the MS Windows operating platform & MS Office products (Word, Excel, and PowerPoint) and Internet functions.

Responsibilities included the overall technical and administrative oversight of all Histology operations and staff. Administrative duties encompassed daily workload scheduling, employee evaluations and development, competency testing, continuing education, departmental budgeting, capital equipment forecasts and expenditures and payroll. Technical and operational responsibilities encompassed complete Quality Assurance oversight of all technical and operational aspects of the laboratory that included the correct and proper preanalytical sample preparation, routine and advanced Histology diagnostic testing procedures and microscopic evaluation of the completed testing for approval, “prior” to being distributed for diagnostic interpretation, analytical interpretation of test results, Periodic assessment of current implementation and upgrading of test procedures, and the periodic amendment of laboratory manuals as regulated by the JCAHO, CAP and ASCP. In great part due to my inherent understanding and proficiency in the implementation of various other integrated instrumentation systems and having an inherent passion and working comprehension within the IT sector I was designated as Administrator of the AP module of the newly purchased Cerner Pathnet LIS responsible for the database development, algorithm and functional template design, all aspects of the implementation and software validation process, pre go live parallel testing, and all user training. My success in accomplishing this major project, in addition having sole responsibility for the development of a new state of the are AP/CP laboratories, I directly participated from the initial blueprint designing, providing extensive feedback, and providing final approval. In addition, I was responsible for the selection and purchasing of all necessary capital equipment. I consider the successful accomplishment of both of these major projects to be highlights in my career.
